Group Dental

You can now receive terrific group dental benefits regardless of where you work.
If you are a member of an eligible group, you may enroll and be guaranteed acceptance in our special Group Dental Plan. This unique program is available for each member of your family. Premiums can be automatically deducted from your account on a monthly basis.

There are four different types of expenses covered by the Group Dental Plan.

• Type I
  Routine exams, cleanings, fluoride treatments for children under 14, and some emergency treatment.
• Type II
  X-rays, lab tests, simple extractions, restorations, antibiotics, and local anesthesia.
• Type III
  Inlays, onlays, crowns, oral surgery, root canals, and periodontics.
• Type IV
  Orthodontics, bridges, dentures, and denture and bridge repair.

All services are paid for on the basis of what is medically necessary, and reasonable and customary for your area. We will pay 80% of the reasonable and customary charges under Type I, and 50% of the reasonable and customary charges under Type II, III, and IV.
During the first three years of coverage, there is a deductible on all types of coverage. The deductible is $50 per family member per year. After three continuous full calendar years of coverage, the deductible is waived. In short, the longer you are on the plan, the better it becomes.
